Eligibility

Must provide DD-214 or Certification of Military Service (NA Form 13038). Must already possess plates with the Symbol of Access or apply for them simultaneously. Limited to one set of Symbol of Access plates.

$28.75 initial purchase (plate number assigned by DMV). $0 annual renewal. Personalization not available.

How to apply

Mail only via Application for Custom Plates (CP-30) form. Not available online, by phone, or in person. Contact the Custom Plates Unit at 518-402-4838 for details.
