Eligibility
At least 50% service-connected disability certified by the VA. Honorable discharge required. Unremarried surviving spouse may continue to use the DAV plate on a vehicle previously titled to the veteran (new applications post-death are limited).
Covers
Special "D.A.V." license plate; option to include international accessibility symbol if also physically disabled.
Veterans not wanting the DAV plate may purchase regular tags for up to 2 vehicles (1-ton capacity or less).

Submit Form 751-J to Service Oklahoma.
