Ohio resident veteran who is permanently and totally disabled (100% service-connected), or former POW. Honorable discharge required.

Note
Licenses are approved for a 5-year period but are valid only for the current license year (expiring the last day of February). Must obtain new licenses each year within the 5-year approval period.
