Eligibility (per 625 ILCS 5/3-609(b)): U.S. Armed Forces veteran with USDVA proof of service-connected disability declared to be 50% or more, whose disability does NOT qualify the veteran for standard disabled-person plates or decal under 625 ILCS 5/3-616.
Benefit
First set of plates issued at no cost.
How to apply
Apply at a local Illinois Secretary of State office, or mail USDVA verification letter to Secretary of State, 501 S. 2nd Street, Howlett Building, Springfield, IL 62756.
