Eligibility (per 625 ILCS 5/3-609(b)): U.S. Armed Forces veteran with USDVA proof of service-connected disability declared to be 50% or more, whose disability does NOT qualify the veteran for standard disabled-person plates or decal under 625 ILCS 5/3-616.
